We will be happy to send you an admission packet. Each packet includes a viewbook, an application for admission, additional forms and information to help you complete the application process.

Although applications for admission are accepted year-round, our primary admission season ends in April. Selection is based on standardized testing, classroom teacher recommendations, a current report card, school transcript and a family or individual visit to the campus.

Kindergarten Application Checklist

  Fort Worth Academy will contact the preschool for a recommendation. (The preschool recommendation form is not included in the information packet.)

Grades 1-8 Information Packet Contents

• Application for Admission – Complete the application form and mail it with a non-refundable $75 application fee to the Admission Office. ($25 reapplication fee)
• Classroom Teacher Recommendation Forms - All teacher recommendations are confidential. Complete and sign the parent waiver in the boxed area of the teacher recommendation form(s). Give the recommendation(s) to your child’s current Language Arts/ English or Mathematics teachers to complete. We ask the school to fax or mail all recommendation forms directly to Fort Worth Academy.
• Request for Transcript – Complete and sign the Request For Transcript form and take it to the office at your child’s school. The school will fax or mail the transcript, report card and additional information directly to Fort Worth Academy.
